Distribution of Vagal Structures at the Distal Exophagus

Abstract
Examination of the distribution of vagus nerve fibers from the diaphragmatic level to the esophagogastric junction level in 80 adult Korean cadavers revealed considerable individual variation.
One anterior vagus nerve and posterior vagus nerve each were simultaneously found in 40 cases(50%). More than one anterior and/or posterior vagal structures were found in the other 40% cases. The number of anterior and posterior vagal structures
were
different in 28 cases at the diaphragm level and in 21 cases at the esophagogastric junction level.
There was 41 distribution types in 80% cases. In the most commonly encountered type, one anterior vagus nerve lay to the left of the midline of the esophagus at the diaphragmatic level and at the midline at the esophagogastric junction level, and
one
posterior vagus nerve lay to the right of the midline of the esophagus between the diaphragmatic level and the esophagogastric junction level. This type was seen in 15 cases. The next most common type was found in 8 cases, while other types were
found
in 6 cases and 5 cases, Two types were found in each 3 cases, 5 types in each 2 cases. Thirty different types were found in 30 cases.
The pattern of the vagus nerves around the distal esophagus is very important to surgeons performing truncal vagotomy. However because the variations of the vagus in this area were so diverse, every fibroareolar structure found in this area
should
be
transected in a complete vagotomy.
